Oakley Forman, age 63 departed this life on Friday, October 27, 2023 at his home. He was born on Thursday, January 28, 1960 in Manchester, Kentucky to Rex and June (Allen) Forman. He was a former supervisor at Jackson WWS and was a member of the Church of God.
He leaves to mourn his passing his children: Jeremy Forman and wife Lynn and Andrew Forman and wife Courtney; these grandchildren: Kat Kreimer, Cambria Forman, Colton Forman, Andi Grace Forman, Elijah Forman and Adaleigh Forman. Also surviving is his special friend: Wanda Gilbert and these sisters and brother: Freida Bellew, Billie Jo Smallwood and husband Jamie and Velman Forman and a host of many nieces and nephews.
He is preceded in death by his parents: Rex and June Forman and his sister: Brenda Forman.
A Memorial Service will be held at Church of God Worship Center on Saturday, November 18, 2023 at 11 A. M. Rev. Joey Burns will be officiating.
